In my opinion it depends which sort of grass it is, if it's couch grass I would try and remove as much as you can, if it's lawn I would just cover it with soil

You could always lay a weed membrane down
This is what I did when I made my raised beds on the lawn

but 5inches deep wouldn't be enough root room, so weed membrane is not a good idea in this instance.

I'd fork the lawn up and turn the grass roots upwards and then fill with compost/soil mix. As long as it not couch grass it shouldn't grow back.

I'm with mumofstig on this one.

I'd be inclined to lift all the turf, then turn it upside down so the grass dies down, then you can put some soil on top.

id lift it just to be on the safe side. while your there you can fork over the soil and work a bit of manure in before you fill the bed

Yup, the best thing to do is cut the turf off in strips, fork the ground over well and add something like BFB and rake level, then lay the turf strips back over the soil but upside down. Cover with your compost and within a year the turf will have turned into nice loam.

I would lift the turf and stack it somewhere grassy sides to grassy side.  It will eventually make some good loam.  Then dig the soil as much as you can and add sharp sand if it is clay.  Then put the soil on that.  The better you prepare, the better it will be. :)
Cheese makes everything better.
