Pinching out creepers

I am growing Orange Queen squashes, also called Japanese pumpkins. They roast beautifully and taste like chestnuts. Anyway, a book I have says I should pinch out the creepers when they get to two feet long. Does that mean I just cut the creeper a few inches above the last fruit? If I have fruit on the creeper above that should I just leave it?
