yoke out of the vent

My 18 month old leghorn has yoke coming out of her vent, no prolapse of her insides muscles, she looks ill (sleeping standing up) and is very still, any advice please?

all i can say is a trip to the vets .. some one better should be along with better advice or say the same thing VETS
