Sutton Broad beans holes in leaves

I have some Sutton broad beans that have been excellent and healthy all along. However, a lot of the leaves near the top of the plants have developed small oval holes. There is no sign of anything eating them and only a tiny number of black fly which I have killed.

Some of the beans are big and healthy and some are small,  grey looking and misshapen. Others dry and fall off before they develop.

Can anyone suggest what could be the matter ?
