It will kill your pears if you don't do something about it.

Pear rust is weird in that it needs both pears and junipers to survive and continue to be a pest.  It spends part of its life on one, and part on the other.  So if you have any junipers nearby either get rid of them, cut out the infected parts or treat them the same way as your pear (as follows)

1.  Practise really good hygene.  Burn, take to the tip of put in the rubbish any fallen leaves.  DO NOT put them on the ocmpost heap.

2.  Spray with a fungicide that is marked as being suitable for pear scab and powdery mildew (I don't think there is anything out there specifically for pear rust)
Good luck
John

KILL, KILL, KILL?

Are sure , you are not too dramatic there, John?

I was just wondering to spray or not to spray...

As it's a young tree, however, as you say, we seem unprepared for this newly all over the place thing and no special treatment seems to be available.

Might as well try garlic? :?

Gobs, as long as the host trees are around, so will your pear rust problem. I haven't found garlic to be of any help with it. BUT, you could try the fire blight treatment I use, which I've sometimes used on flower rust. Spray the whole tree, including bark, with 1 part white vinegar to 4-5 parts clean water. Do about once a week and see if it helps by next month. Don't use this spray when flower buds are out.
