Both of my plots were covered with couch grass when I started.  I first 'sliced' the top layer off with a spade and then dug it over with a fork.  Take out major bits, add some manure, dig in and plant.  Some of my raised beds are not great to be honest and I still get the grass showing through, but it is getting better year on year.

I've read that drowning it and then watering with the resulting solution will inhibit the growth of other couch. Don't know if this will work but might be worth a try in a test area (as there was no mention if the liquid could also affect other plants   :mellow:).
