CHUTNEY FAILURE / ADVICE NEEDED

Hi there
I'm hoping for some advice after my disasterous attempt to make some chutney/pickle.

I made it last week and put it on the shelf. This week it was growing mould on the top and I have binned it.

I would say that my methods are haphazard but I've made it on several occasions without problem.

What would be the main cause for the mould to form so quickly?

This is the first time I have used kilner jars for the purpose.
Could it be that I haven't added enough vinegar?

How did you sterilise the jars? :)
