can hens have piles?

That is a prolapse and can be VERY serious.

Piles cream will help to reduce the swelling (after making sure the tissue is clean).  You may be able to gently ease it back inside then.

She will need to be stopped from laying eggs or it will come out again.  Reduce feeding to about a quarter of what she usually eats and keep her in very low light.

My choice would be to get a vet to look at her in case any of the tissue has become necrotic or infected.

forgot to add - keep her away from the other hens as they will more than likely peck at the area and draw blood
