Plum Tree - help needed to identify a problem

My plum tree is around 4 years old, it has loads of fruit on but the leaves have yellow spots on and is definitely looking poorly.  Does anybody know what could be the problem?

Have you got any photos please, as yellow spots could be so many things including Cherry Leaf spot- which is a fungal infection.
Very often quite lost- would be more lost if I could work out where I was!- But always find my way home.....
